The issue is not just policy it is the lack of a required validation layer before decisions are executed. Without verifying record integrity, outcomes cannot be trusted or defended.
Implementing a validation-before-enforcement standard would reduce harm, improve accountability, and strengthen public trust across all administrative systems.
